Myalgic Encephalomyelitis, also known as Chronic Fatigue Syndrome (ME/CFS), causes profound fatigue that is not relieved by rest, along with post-exertional malaise — a worsening of symptoms after activity that can last days.

People with ME/CFS may be eligible for PIP where this affects daily living or mobility.

The key issue is what happens to you during and after activity, not just at rest.

Post-exertional malaise and PIP

A common mistake is describing only what you can do on a good day, or straight after resting. PIP guidance requires assessors to consider whether an activity can be repeated:

  • safely
  • to an acceptable standard
  • repeatedly
  • within a reasonable time

For many people with ME/CFS, an activity that seems manageable once may trigger a crash lasting days — this is directly relevant.

ME/CFS and daily living

  • preparing food, especially standing or multi-step recipes
  • washing and dressing without becoming exhausted
  • managing medication and appointments around energy levels
  • concentrating enough to manage money or read complex information ("brain fog")
  • engaging with other people when cognitive fatigue is severe

ME/CFS and mobility

Walking or standing can trigger post-exertional malaise even where the distance itself seems short. Relevant factors include:

  • how far you can walk before symptoms worsen
  • whether walking now leads to a crash later
  • how long recovery takes afterwards
  • whether you need to pace activities across the week to manage energy

  • GP records and ME/CFS specialist clinic letters
  • an activity or symptom diary showing crashes after exertion
  • occupational therapy or pacing programme records
  • statements from people who see your day-to-day fatigue

What if I can technically do a task once?

Being able to do something once is not the same as doing it reliably. If it triggers a crash or cannot be repeated, that should be explained clearly.
